5. At the very beginning of LtoJ seminars participants are asked to answer ten questions. Attached are the questions and the responses from 1266 teachers and administrators. Further, seminar participants are asked to rank the ten root causes from biggest problem to least problem and these rankings are placed on a control chart to determine which are special. The control chart showing three root causes as signficant problems is the last slide in the presentation.
